What is Continuing Healthcare Fast Track?

When an individual’s health is rapidly deteriorating – and when they are considered to be approaching the end of their life (known as a ‘terminal phase’) – they may be eligible for fast-tracked NHS Continuing Healthcare.

Fast track paperwork should be completed by a healthcare professional who is involved in the individual’s care and able to make a diagnosis and prognosis.

If they recommend that the person is fast-tracked for Continuing Healthcare – because they meet certain specified conditions – the local Clinical Commissioning Group (CCG) should accept and process the request immediately.
